The Gateway Team-Based Care Teaching Clinic is an innovative primary care teaching clinic where learners and health professionals work together in partnership with patients, who are at the centre of the team and actively shape their care.

Care and learning happen together in real time. Patients, learners, and experienced clinicians work as a team, continuously improving how care is delivered and how we learn from one another to achieve better outcomes. As a primary care clinic, you will work with a team of practitioners, building strong relationships that support your health and wellness throughout your life.
